The project "Transformation of Day Care Centres for People with Disabilities through Community Participation" was implemented in five communities in Bosnia and Herzegovina from 2011 to 2013 with the goal to promote equality of every person through supporting centres for people with disabilities to become sustainable inclusive centres, open to all community members. 

The report presents final evaluation findings which are based on analysis of qualitative and quantitative data collected during project-end and baseline assessment. The report is structured to follow three main areas of change which occurred as an outcome of the project activities; 

- Strengthening Centres' organizational resources and capacity, 

- Enhancing Centres' openness and inclusiveness;

- Initiating Centres' self-sustainability through development of income-generating activities.
